NOTE: All California courts use the same basic set of forms. But some courts have special, local forms, too. To see if you will need any special, local forms, contact your court clerk or check your court’s website. The forms may be posted on their site. If not, the site will list the address and phone number of your local courthouse.

Request for Elder or Dependent Adult Abuse Restraining Orders EA-100 Use to start the court case to get an elder or dependent adult abuse restraining order.
Notice of Court Hearing EA-109 Tells you when your court date is.
Temporary Restraining Order (CLETS — TEA or TEF) EA-110 Tells you which, if any, temporary restraining orders the judge signed.
Confidential CLETS Information CLETS-001  Fill out when you file your request for a restraining order. Gives information about your case to all law enforcement agencies through a computerized database.

What Is “Proof of Personal Service”? EA-200-INFO This form gives you information about serving the court papers in person.

Proof of Personal Service EA-200 Use to prove that you gave the person to be restrained a copy of the petition, notice of court hearing and temporary restraining order in person.
Proof of Service of Order After Hearing by Mail EA-260 Usually, the protected person must serve papers in person, but in some situations, the restraining order after hearing, on Form EA-130, you are legally allowed to serve by mail. In that case, you can use this form.
Elder or Dependent Adult Abuse Restraining Order After Hearing (CLETS — EAR or EAF) EA-130 If the judge grants any restraining orders, he or she will sign this form.
How Can I Respond to a Request for Elder or Dependent Adult Abuse Restraining Orders? EA-120-INFO Serve on restrained person with restraining order request forms (Form EA-100, EA-109 and EA-110), and with a blank EA-120.
Response to Request for Elder or Dependent Adult Abuse Restraining Orders EA-120 Leave blank and serve on restrained person with the TRO (Form EA-120).
Proof of Firearms Turned In, Sold, or Stored EA-800 Leave blank and serve on restrained person with the TRO (Form EA-120).
